Wellness requirements
To stay in the Wellness program, you must:
- Get all your primary care at your designated 5 Star Center. If you change your 5 Star Center, you must call Health Services to let them know.
- Continue to see your 5 Star Center PCP at least once every 9 months.
5 Star Centers provide excellent care at the lowest cost to you
5 Star Centers are doctors’ offices that meet high quality standards set by the National Committee for Quality Assurance (NCQA).
You pay a $0 copay for doctors’ visits and other services* at most 5 Star Centers |

*Except for high-tech radiology services like MRIs and CAT scans. **Urgent care at Summit Health 5 Star Centers has a $40 copay.

5 Star Wellness participants can save hundreds of dollars each year on their prescription drugs
You may be eligible for the 5 Star Wellness program and receive discounts on all your prescription medications if your doctor has diagnosed you or your family member with at least one of the following conditions.
Asthma
Chronic obstructive pulmonary disease (COPD)
Diabetes
Heart Disease
Hypertension
Peripheral artery disease (PAD)
Stroke
