Physical Therapy (PT) Multiple options to relieve joint and muscle pain—with and without a copay |
In-Person PT
Your health plan covers a maximum of 30 in-person PT visits in a calendar year. You must see an Anthem-network physical therapist, and preauthorization is required.
FREE Spear PT at the Union Health Center
If you go to UHC for primary care, you can also go to Spear’s UHC location and pay a $0 copay for physical therapy (only at this location).

Other Anthem-network PT
To find more options for care, use the Find-a-Doctor tool on the 32BJ Member Portal app or call 32BJ Health Services at 866.230.3225, Monday-Friday, 8:30 am-5 pm.
You pay a $40 copay for PT you receive in an office setting to manage chronic pain or before having surgery. Questions? Call Health Services.

Virtual PT with Hinge Health
FREE virtual physical therapy from Hinge Health is available to eligible 32BJ members ages 18 and older who experience muscle and joint pain.
- You can choose to have your appointment with a Hinge Health physical therapist in English or Spanish using the Hinge Health app
- Your physical therapist will create a personalized treatment plan and give you easy exercises to do in between your appointments

Generic and brand-name prescription drugs with low copays
The Health Fund gives you prescription drug coverage through OptumRx.
- You can fill most prescriptions at any participating pharmacy like CVS
- You must fill longer-term prescriptions (for medicine you will take for more than 60 days) at CVS or through the OptumRx mail service

You pay a $0 copay for dental exams and cleanings with a Delta Dental dentist—twice a year
Did you know heart disease, diabetes, and Alzheimer’s can be caused, in part, by poor dental health?
Your comprehensive dental benefits through Delta Dental cover X-rays, fillings for cavities, tooth extractions, crowns, dentures, bridgework, and much more.

Important: When you see a medical doctor, including at 5 Star Centers, for an eye appointment, it is covered by your Anthem medical benefit.
Some eye health professionals accept both Davis Vision (for vision appointments) and Anthem (for medical appointments), but there are no Davis Vision optometrists at 5 Star Centers.
