Your Lantern ID card does not replace your purple Anthem ID card, which is why you need both cards when you see a Lantern surgeon.

Spear PT at UHC: $0 PT for UHC primary care patients

If you are a UHC primary care patient, you can go to Spear at UHC and pay a $0 copay to receive in-person PT before and/or after surgery (up to the maximum of 30 visits in a calendar year).

Need Bariatric, Joint Replacement, or Spine Surgery? You MUST go to Lantern. Your surgery copay is $0.

The following surgeries are ONLY covered with a world-class surgeon who is in network with Lantern Surgery Care:
  • Bariatric
  • Joint Replacement (hip, knee, ankle, shoulder)
  • Spine
Important: Even if you went to Mount Sinai before, you MUST go to a Lantern-network surgeon for these surgeries. Otherwise, you will have to pay the entire cost of your surgery—which could be tens of thousands of dollars.

If your Lantern surgeon recommends physical therapy (PT) before, after, or instead of surgery, you can choose virtual or in-person PT to fit your busy schedule.

Virtual, or online, PT is easy to do from home with the Hinge Health app. If you are in pain and can’t get to an in-person PT appointment, virtual PT with Hinge Health would be a great choice for you.

Hinge Health uses advanced technology to create a personalized physical therapy program that you can do for 15 minutes two or three times each week.  Your benefit covers 12 months of unlimited virtual PT with Hinge Health before and after surgery and your copay is $0.

In-Person Physical Therapy

Your health plan covers a maximum of 30 in-person PT visits in a calendar year with an Anthem in-network physical therapist, and preauthorization is required.  Here’s how it works:
  • After SPINE surgery: You can receive in-person PT with a $0 copay during the first 120 days after your surgery
  • After JOINT REPLACEMENT surgery: You can receive in-person PT with a $0 copay during the first 50 days after your surgery
    • If you do not use up your 30 visits in the first 50 days: You can continue to receive in-person PT with a $40 copay
  • To manage chronic pain before surgery: You can receive In-person PT with a $40 copay
